James O'Keefe is a long-time supporter of the Massachusetts Clean Elections Law and is proud to run as a Clean Elections candidate. However, since the legislature eliminated this system, James O'Keefe is attempting to qualify under the previous public funding system. The campaign will accept contributions from individuals up to $500.

• I am at least 18 years old.
• I am a US citizen or permanent resident with a green card.
• I am making this contribution from my personal funds and on no one else's behalf.

1. Make out a check for no more than $500 to the James O'Keefe Election Committee;
    
2. Please provide your occupation and employer if your contribution is $200 or more;
